REFERRAL PROGRAM TERMS & CONDITIONS
This Agreement is made between KODAK Lens Vision Centres operated by eyeTrust.ca Corproation (the “Company”) and you, the individual who is referring a friend or family member to the Referral Program (the “Advocate”) and/or you, the individual who has been referred by a friend or family member to the Referral Program (the “Referee / Friend”.)
These Terms and Conditions (“Terms”) set out the provisions pursuant to which the Company will provide a Referral Program to you, customers, potential Friends & potential Advocates (the “Referral Program”).
The Advocate & Friend understand that the Company has the sole authority to change, modify, amend or cancel any referrals, rewards, promotions, terms related to the Referral Program or the entire Referral Program at our sole discretion, at any time and the Company will not be liable for any related loss of service, value or reward. The Company may decline a referral or reward at anytime without giving a reason and / or remove your eligibility from our Referral Program.
Advocate Terms:
Advocates cannot refer themselves. A Friend that is an existing customer or user of our services, or someone who has already been referred by another Advocate will not be eligible for the Referral Program and therefore the Advocate will not earn a reward for these referrals. The Referee / Friend must be new to Kodak Lens Vision Centres (has never previously made a purchase or had a service rendered at any Kodak Lens Vision Centre in Canada). By participating in our Referral Program the Advocate acknowledges and agrees that the Company’s determination will be final and binding with respect to the Advocate’s eligibility for any reward.
After each successful referral, the Advocate can earn a choice of $25 e-gift card or $50 off their next purchase of eyewear at regular price. Eyewear includes any frame and lens package or non-prescription sunglasses. Terms for a successful referral are outlined below. Advocate Rewards can be used for future purchases only and can not be used for any outstanding balance on an existing purchase. The reward may not be combined with any other offers or promotions. Rewards cannot be used towards MiyoSmart, Stellest, MiSight, Abiliti, Essilor, Zeiss, Nikon or other specialty contact or spectacle lenses. Rewards cannot be used towards no-fault warranty, custom upgrades, or dispensing fees. Full details will be provided when reward is given.
The more you refer, the greater amount for your e-gift card you may accumulate. For example, if you refer 2 friends, you can get $50 in a e-gift card or $100 off your next purchase of eyewear at regular price. Once a choice has been made, it is final. A list of e-gift card options will be provided to Advocate after every successful referral. The reward cannot be applied to invoices already created and cannot apply to services, contact lenses or nutraceuticals. Referral Rewards must be redeemed within 2 years.
Store credit may be transferred to another person under the permission of the Advocate who received the reward.
By participating in our Referral Program the Advocate acknowledge and agree that the Company’s determination will be final and binding with respect to the Advocate’s eligibility for any reward.
Referee / Friend Terms:
In order for an Advocate to be eligible for the referral reward, the Friend must be a new customer to the Company and meet the requirements by purchasing eyewear (frame and lenses), contact lenses, or sunglasses. If the Friend was referred by more than one Advocate, the Advocate who referred the Friend first will receive the reward.
The Friend will receive a minimum of 10% off their purchase of eyewear (frame and lenses) or contact lenses. If another eligible current promotion offers a greater discount, this may be applied, but cannot be combined with the 10% off. The 10% off Friend discount is applicable only on a one time basis for the Friend’s first purchase at regular price and cannot be saved or used for future purchases.
By participating in our Referral Program the Friend acknowledge and agree that the Company’s determination will be final and binding with respect to the Friend’s eligibility for any reward.
Advocate & Referee / Friend Terms:
Advocates and Referees / Friends cannot refer each other to be qualified for the Referral Program. The Advocate and Referee / Friend must provide their phone number and e-mail address to participate in the referral program. All communication will be sent by email. As such, Advocates must respond to email to receive rewards. Each Advocate’s referral and contact information will be validated by a Kodak Lens Vision Centre team member to be eligible for rewards. To provide your contact information, please visit us in clinic and inform our team or contact us online through web chat, contact forms, or email.
Additional terms and conditions:
- Advocates cannot receive a reward if their referee / friend:
- Is an existing Kodak Lens Vision Centre customer
- Did not purchase eyewear, contact lenses or sunglasses
- Purchased eyewear using social assistance (ODSP, OW, IFH or CHC)
- Purchased eyewear using a special discount (discount outside ongoing and seasonal promotions)
Advocates that refer students (who use their student benefits) may receive a $10 e-gift card of their choice when the student makes an eyewear, contact lens or sunglasses purchase.
Store Credit (Past referral program terms and conditions):
This term only applies to participants of our referral program dated before February 1, 2024. Advocate rewards as store credit can be used for future purchases only and can not be used for any outstanding balance on an existing purchase. Store credit may only be used toward regular priced complete eyewear (frame and lenses) or prescription and non-prescription sunglasses. The reward may not be combined with any other offers or promotions. Full details will be provided when reward is given.
