INTRODUCTION & TERMS OF USE

1. Collect points to earn rewards and reach reward levels.
2. Each purchase gives you points.
   • Level 1: Spend $1 or more and earn 1% back (ex. Spending $100 earns you 1 point). The first scanned purchase always starts at Level 1.
   • Level 2: Spend $300 or more and earn 2% back (ex. Spending $300 earns you 6 points). Purchases are cumulative, for example, if your first scanned purchase was $200 and your second scanned purchase was $600 then you will automatically be upgraded to Level 3 ($600 +$200 = $800).
   • Level 3: Spend $700 or more and earn 3% back (ex. Spending $700 earns you 21 points).
   • Level 5: Spend $1200 or more and earn 5% back (ex. Spending $1200 earns you 60 points).

4. Loyalty Program reward chart:
   1. When you have earned 25 points, you can redeem to claim a $25 gift card of your choice. Vendor gift card list is available on request. The customer can save points and redeem the reward all at once (for example, if a customer obtains 100 points, then can claim $100 in e-gift card rewards).    2. When you have earned 100 points, you can redeem the 100 points for $125 off on your next prescription or non-prescription eyeglass order, prescription or non-prescription sunglasses order, and/or any supply of contact lenses. This discount will directly apply on the order once the points are successfully redeemed. Order must be $125 or greater in value. The full $125 must be used in one visit where remaining unused value can not be saved as credit. The customer can accumulate points to redeem all at once (for example, if a customer obtains 500 points, then can claim 5 x $125 = $625 discount).
5. Rewards cannot be exchanged, returned, replaced or bought with cash.
6. Cards cannot be transferred and combined with other cards.
